End of life care
St Denis Lodge is proud to announce that we have achieved the Platinum Award for End of Life Care which has been accredited by the Gold Standards Framework. This award celebrates the sustained excellent practice of care homes that demonstrate their credentials for the second, third or fourth time and are recognised with the Platinum Award.
This is a process ensuring a better and continuing standard of care for residents, allowing each individual to prepare for their final days in accordance with their own and their families wishes.
As part of your admission process residents are invited to an Advance Care Planning Discussion which can involve other family members. You will have the opportunity to discuss all aspects of your life at this time and what elements of care are important to you. Also, what you would like to happen in the future and most importantly is there anything that you worry or fear about happening. This allows you to take control of your own future by making us aware of your choices. You and your family will be supported and treated with dignity and respect at all times. We will be there to offer advice and guidance.
We find that as a result of the Advance Care Planning, treatment and care can be planned and co-ordinated with our local Medical team. This means fewer crisis admissions to hospital allowing you as an individual to be cared for in the home by the staff that you have grown to know and trust.
